Vision testing—not a substitute for a full DOT physical.
A DOT-related vision evaluation may assess distance vision, visual fields, and the ability to recognize traffic-signal colors. The tests and documentation depend on the request from your certified medical examiner and the requirements that apply to your situation. Bring the exact paperwork rather than relying on a general description.
This service concerns the vision component and related eye-care documentation. It is not a promise of a complete DOT physical, a medical certificate, or clearance to drive. A certified medical examiner makes the commercial-driver medical certification decision. Please confirm the practice’s Louisville availability, testing scope, and form requirements before scheduling.
Will this appointment renew my DOT medical card?
A vision evaluation alone does not renew a DOT medical card. Your certified medical examiner must complete the overall medical examination and certification process. Confirm which vision information they need before your appointment.
A little preparation helps.
- Bring the form or written request from your medical examiner.
- Bring your current driving glasses or contact lenses.
- Provide your examiner’s contact details and any relevant deadline.
Your questions, answered.
General patient education, not individual medical advice. Tests, treatment, and follow-up depend on your examination.
