Bring the current form supplied by your school, licensing agency, or medical examiner. Different forms can require different examinations, tests, and documentation.

Tell the practice what you need and any deadline when booking. An examination does not guarantee licensing approval, school acceptance, or commercial-driver certification.

Have a communication preference, mobility concern, or another access need? Call before your visit so we can discuss your questions and help you plan.

Talk with the practice

Keep health information private.

Ordinary email is not a secure patient portal. Do not send medical records, diagnoses, dates of birth, or insurance numbers through the appointment email helper. Call to discuss a suitable way to share records.

Some symptoms should not wait.

Sudden vision loss, severe eye pain, new flashes or floaters, a dark curtain in your vision, or an eye injury needs prompt medical assessment. Do not wait for an email reply. For a medical emergency, call 911.
